Spoke 10 aims to provide advanced methods and tools to support the governance of the Tuscan Health Ecosystem and provide evidence to bridge the gap between innovation and its large-scale implementation, thereby ensuring a significant improvement in the health and well-being of individuals and communities. The activities of Spoke 10 focus on five main areas of intervention:

1. From Prototype to Large-Scale Testing: This process involves the social, clinical, and economic validation of innovations within the context of the population and the healthcare system. The goal is to advance new solutions and technologies from the prototype phase to actual large-scale testing to assess their impact and long-term sustainability.
2. Coordination and Integration of Healthcare Services: Improving continuity of care and creating public value through a coordinated approach that integrates various healthcare services. This aspect is crucial for ensuring that patients receive continuous and integrated care, reducing fragmentation and discontinuities in care pathways.
3. Towards an Integrated Care System: The focus is on individuals with chronic diseases, multimorbidity, and frailty. The aim is to develop a system that effectively responds to the needs of these population groups through personalized solutions, multidisciplinary approaches, and polypharmacy.
4. Community Engagement and Social Innovation: Promoting local community involvement in the development of innovative solutions for health and well-being. Inclusion and active engagement of people are key elements to ensure that innovations are relevant and sustainable in the social and cultural context in which they are implemented.
5. Migratory Trauma and the Tuscan Healthcare Industry: Addressing challenges related to health and cultural mediation for individuals who have experienced migratory trauma. This area of intervention aims to develop healthcare solutions that are sensitive to different cultural experiences and the specific needs of migrants, improving access to and effectiveness of care.

An additional crucial element of the project is the implementation of innovative technologies for telemedicine, including connectivity services, to support integrated and patient-centered healthcare across Tuscany. Some inner areas of the region, such as Garfagnana, the Elba Island, and Casentino, are involved in trials to test these new solutions in specific contexts.
Finally, the project places strong emphasis on networking and integration between the non-profit sector and the healthcare system, to foster the implementation of innovations in care pathways. Collaboration among various stakeholders and the creation of support networks are essential for promoting systemic and sustainable change in healthcare.

The Rector of the Sant’Anna School of Advanced Studies in Pisa, Sabina Nuti, presents the activities of Spoke 10, which focus on public health.
She particularly emphasizes digital healthcare and the proximity of care services to bring social and health services directly to citizens’ homes.
